Maybe Greg Palast was right?
It looks like there were questionable voter roll purges in at least one location (see also Palast's skyfall panic from earlier in the week). Currently, four Senate races are within a percent or so with the Republicans up. Instead of celebrating victory, I wonder if the Democrats should start challenging close elections and demanding to see provisional ballots.

I did some quick googling and couldn't find any reports of this kind of thing happening anywhere else. The best numbers I could dig up on the use of provisional ballots in general were that 10,000 provisional ballots were cast in Ventura County as compared to 240,000 votes tallied normally. That's about 4% of the votes, 3% if you count the 80,000 absentee (easily forged) ballots cast in the same county.

There also were machine accuracy problems. It's a shame that those are to be expected.

More: Uncounted California votes "in millions".
